I think the advice he gives is pretty much for big games with large stacks, where going all-in is a huge move, much more powerful than we can imagine playing low relatively high blind/low buyin games on the internet.
I've also read Doyle himself quoted as saying (and I paraphrase) "I'm not so aggressive any more because after people read my book they started to call me more and I had to show them a real hand".
I wouldn't advocate this approach on the internet unless you're playing with someone weak and/or is betting scared money (i.e. the relatively tame all-in will frighten the wits out of them). I would think that the playing style is hugely more effective face to face, as well.
You can tell which people have read Doyle's section and got straight onto the internet. They're called 'Maniacs' and all you have to do is sit tight and wait for a big hand.
If you were playing $1/$2 NL and you had a stack of $1000 instead of $100, you'd be a lot less likely to call an all-in headsup, and this is the way I reckon that NL should be played in its purest form.
However, balls of steel (and the knowledge of where you can find a game like this) required!
