For me the Prima Push happens most often with a shorter stack and in the following:
1) I have TPTK and bet the flop, he calls. Turn ck/ck. River I check (giving him an opportunity to bluff) and he instapushes with 2nd pair, etc. Board is fairly dry.
2) I have TPTK and bet the flop (there's two to a flush), he calls (or better he check/calls if I have position). Turn is no flush and I bet, he calls (or check/called). River is no flush and he instapushes.
