We've already identified the leaks of ignoring flop min-raise warning signs and checking reflexively when you hit a monster to 'trap' rather than thinking about how to build the pot(which alone should add about 50% to your win-rate if implemented).
And the other reason to raise is if you have a hand that is likely ahead of the other players' ranges and you have position on them - that's a mandatory raise for value. I would aim for 22/9 at first (going from 6% to 13% is going to be too much of a stretch).
And of course you should play less tables - playing half the number (4) would give you much better table selection, much better reads and help you become a much better player so you can play and beat the higher levels sooner.
