This usually 1 of 2 types of hands in my limited experience.. AA/KK or a smaller PP that they don't want to see a flop OOP with. Although I'd consider how long he's been at the table and how active he has or hasn't been.. If he's doing nothing but folding the whole time I'd be pretty apprehensive about calling anything other then AA/KK.

1. AA or KK
2 yes if he's that short I'd open up to AK, AKs AQs, QQ, JJ maybe AJs.
3. I don't think so (assuming we're still on a $132 stack not 68)
