Australia/New Zealand - I've got some friends from these two countries and I also met a number of travelers from these countries while I was abroad last year, and these people kick a$$. They are really chill and love to booze, and additionally I have also heard great things about the beaches and outdoor-sy type $hit you can do.
New York/Miami - If you are gonna go to SF, you might as well flip it to the other coast. These are probably the two most entertaining cities on the East Coast of the USA, though incredibly different.
Jamaica - Apparently is pretty awesome; my cousin went there for 2 weeks a little over a year ago, and he had a blast. Weather/Beaches/Women/Weed/you name it.
You got a lot of places on your list that look pretty awesome and I would like to check out as well (Brazil, Bora Bora, Germany, Greece, Italy, London (England), Sweden).
I only got to go to Amsterdam, Prague, Zurich/Interlocken (Switzerland), Barcelona, Ibiza, Lisboa.
You've got a pretty ridiculous line-up from the looks of it already though

Well, I live in Australia 
So this part of the world is fairly easy to travel around.
As for New York/Miami, I was in the US about a month ago and I went to NYC as well as a bunch of other places (I stayed in San Diego mainly) so i'm not really looking to spend a whole lot of time there. My friend i'm traveling with however hasn't been to the US before and just wants to check it out - I think San Fransisco is the best city there that i've been too (thanks ED!) so I figure we'll just spend a few days there. My sister also lives in San Diego so it'd be nice to see her. He doesn't really want to spend too much time in the US and neither do I, so I think we'll just to the west coast.

The West Coast is pretty amazing, but then again I'm kinda biased (grew up in Bay Area, South San Francisco-ish and now live in San Diego).
Sounds like an amazing trip - just try not to burn out. I did a month in Europe and I felt like a shell of my former self when I returned. You're talking like 6 months to a year or something right?
That $hit will wear you down if you're staying in hostels/two-star hotels a lot.
