Yeah, it sounds to me like you may be a little too loose sometimes OOP and possibly too tight with position. I don't like the calling with any 4 in BB if there are lots of callers already due to your bad position.
For me, in calling a raise, I like to make a quick calculation of what I'm going to need to bet and of how often I'm going to hit that. Setting with nothing else (i.e., big pair with crappy side cards) CAN still be good if, as bri mentions, the implied odds for that particular player are high.
I also think I sometimes get too picky about wrap quality here. Even a 2-holer like 68TJ is still pretty good, particularly ds. The ds I view less in terms of making the flush but in terms of making it a lot easier to draw to straights when the board isn't rainbow.
